Changes to cut paper-based administration and improve patient care are being brought in by the Department of Health and Social Care.
The Department is making technology-driven changes to reduce the amount of times a patient needs to give the same information to different people who treat them.
Patient information will be shared between GP surgeries and the Hospice, while community nursing teams, health visitors and a number of specialist nurses will be added in the near future.
Only the main patient record will be shared, not handwritten notes or comments, while GPs will be able to override the ability to share data if the patient's mental capacity is a concern.
The scheme is part of the Government's digital strategy and commitment to digitally transforming the hospital and health care services.
