More than 200 complaints were made against Noble's Hospital last year.
The figures have been revealed in a report that will be presented at this month's sitting of Tynwald.
208 grievances were filed during the 2015-16 period compared to 196 the year before.
All aspects of clinical treatment was the most complained about category with 85 complaints, while communication and information also attracted negative feedback.
Other areas of the health service fared better, with fewer complaints made against the community services, pharmacies and opticians.
